6.0 YHAK’S APPROACH
6.1 Our methodology follows the following steps
i) Preliminary Social and risk assessment
We facilitate discussions with community leaders and social workers to identify social needs and
risks involved with regard to these social needs. Key high-level information, including personal data
and contact names, are obtained and we solicit individual concerns and work with the individual to
define and communicate expectations.
We take time to thoroughly understand the needs to ensure that the resources put in place support
our intention to rehabilitate and reform key population who abuse drugs.
ii) Planning
During planning we gather information on the current situation and position of the affected
individual, how the individual is trying to control the situation. Through discussions we understand
the needs and how YHAK can facilitate rehabilitation.
iii) Execution phase
At this phase we execute the activities based on the overall strategy and plan in place utilizing the
policies specifically provided for the organization to manage and ensure that goals and targets are
met.
iv) Reporting and communication
At the end of successful operation, our team will meet with the beneficiary to discuss progress
points, compile them to a report to be filled and retrieved on donors or related party’s request.
